Forms for Public Use
This form is for the provision of information regarding excellent customer service and care provided by any employee of ECDC or to provide information in regard to an issue or concern following a call for service to ECDC.
This form is used to submit a records request to ECDC. This form must be completed in its entirety for a request to be fulfilled. The completed form must then be forwarded to ECDC.
The Commission on Accreditation for Law Enforcement Agencies, Inc. (CALEA) requires an annual statistical summary of Internal Affairs Investigations and 911 Citizen Survey to be made available to citizens and department personnel.
